Description
This session will look at one significant transition that occurs for our students at school. For any student, it can be an exciting yet challenging time as they prepare themselves for the next stage in their learning. This particular PL will be focusing on students with a disability transitioning from Year 6 to 7; primary to middle school.
Effective transition processes and practices are based on a broad interpretation of the concept of transition. The key topics that will be discussed are:
* A person-centred approach, taking into account the needs of the individual student and their family
* Advanced planning of the transition
* The use of collaborative processes that build relationships over time
* The identification of adjustments and supports required to ensure that students with disability can access and participate in the transition process with their year-level peers.
