Description
This session will look at one significant transition that occurs for our students at school. For any student it can be exciting yet challenging time as they prepare themselves for the next stage in their learning. This is the time when we have to consider our students with a disability, on how they manage the transition at particular points in their school career. This particular PL will be looking at transition from Year 6 to 7.
Effective transition processes and practices are based on a broad interpretation of the concept of transition. The key topics that will be discussed are:
* Person-centred approach and take into account the needs of the individual student and their family
* Planned well in advance of the transition
* Use collaborative processes that build relationships over time
* Identify the adjustments and supports required to ensure that students with disability can access and participate in transition processes and supports on the same basis as their year-level peers.

Coordinator Inclusive Education
Rebecca Delaney is an Inclusive Consultant for AISWA. Her expertise in Special and Inclusive Education has spanned over twenty five years both in the United Kingdom and Australia. Working in a variety of roles such as a specialist support assistant, teacher, tutor and Head of Department she has gained valuable training and knowledge, working alongside other professionals in Special Education. She has also experience in the field of Gifted Education and Early Childhood and has utilised this in a variety of different contexts in her career.
